Measurable PostSecondary Goals:

These are parent and student statements that declare the student’s future aspirations after graduation. They must contain what the student WILL do; when and to what degree. Example: Upon graduating from High School, John will enroll full time in the auto mechanics program at LSC.
There must be statements for post-secondary education/training and for employment. Independent living statements are required only if appropriate.
Goals must be updated annually.

Courses of Study:

Courses of study is a listing of the classes a student will most likely take and are technically aligned to the student’s measurable postsecondary goals and graduation requirements.
Must list specific courses not just using “electives.” What are the specific elective courses the student will be taking?
The team is required to list two years of projected courses.
Special education instruction can be included in the course of study (reading, functional/behavioral, cooking, community participation, etc).
The team can consider course substitutions and list those in this section.
Update courses of study at each IEP meeting if the courses change from the previous year.
ONLY high school courses (9th -12th+) need to be reflected.

Anticipated Graduation Date:

This needs to be addressed during the student’s 9th grade IEP meeting and each IEP meeting thereafter.
It is the expectation that students will graduate with their same age peers, however for those students that demonstrate a continued need of transitional support they may continue on after their 12th grade year.
This is an IEP team decision and not just a parent determination or student preference.

Transition Services:

This is a listing of all the activities including special education instruction that have or might take place that will help the student to attain their post-secondary goals.
The Transition Activities & Examples Guide (pdf) can assist you with completing the needed sections.
Assign responsibility for the activity or learning. Simply stating “school,” “parent & student,” “county” or “VRS” is sufficient.
Revisit annually.
If previous activities have not occurred as planned, discuss alternate ways of achieving or activities that could be substituted.

Age of Majority:

Student and parents must be informed of the legal changes regarding special education that take place when a student turns 18.
This information must be provided at least one year prior to the student’s 18th birthday. It is therefore highly recommended that information be provided at the IEP meeting when the student is 16 and re-affirmed at the IEP meeting when the student is 17.
Prior to the meeting, complete the Transfer of Parental Rights document on SpEd Forms and print a copy to provide to parents.
Make sure to include a statement on the PWN that the team discussed the transfer of parental rights.
